Boring Health and Safety. Why Bother?

Lots of examples where a lack of health and safety has resulted in serious injuries to staff. One worker losing fingers in a machine, teenager trapped in an industrial washing machine, another worker asked to use machinery he had not been trained to use resulting in him being crushed. Why?!  In 2015 you would think that workers should be able to return from work each day safely and without injury . What's going wrong? Lack of a safety culture? Lack of management supervision? Not sure what you should and should not be doing?
