Health and Safety at Work

I have just read an article in Jan/Feb Safety Express. The article described challenges to the UK's safety record. What do you think of the UK's health and safety record? Personally, I think we do a good job, although there are always things we could do better. Do you have polices and procedures already in place to prevent accidents before they happen?  Or do you hope for the best and then when things go wrong implement a policy. The first is proactive management. The latter is reactive. What ever industry you are in are you a proactive or reactive manager?
